Apply for Long Term Care

You can apply for long-term care through Ontario Health at Home.

Steps to apply

1. Contact Ontario Health at Home. Call 519-631-9907 or 1-800-811-5146 to start your application.
2. We review your application. We confirm that Valleyview is an appropriate placement for your needs.
3. Join the waiting list. Once we approve your application, Ontario Health at Home places you on their waiting list.
4. Ontario Health at Home will contact you when a bed becomes available.

What to know before you move in

• you must use our facility pharmacy
• if you are staying in a basic room, bring your most current Notice of Assessment from your income tax, since you may qualify for a rate reduction. We do not offer rate reductions for semi-private or private rooms
• if you have appointed a Power of Attorney, bring the documents so we can copy them for our files
• pack comfortable, machine-washable clothing. About 7 changes of clothes is a good starting point
• avoid bringing large amounts of money or valuables, since we cannot take responsibility for lost or broken items

We make every effort to help couples live together if they choose. Let us know this preference when you apply.
