  • Two weeks notice is needed prior to the date the certificate is required
  • All requests are subject to review
  • All certificates requests must be made in writing or submitted online using the form above.  No telephone or verbal requests will be prepared without written confirmation.  Requests in writing avoid confusion and the possibility of errors.
